If you are the owner of a business listed in our PPP loan database and wish to have your record permanently removed, complete the form below. Requests are reviewed by an administrator before any record is removed. We may contact you to verify your authority to request removal.

You can find your loan number by searching for your business name and opening the company detail page — the loan number appears in the URL.

Used only to confirm when your request has been processed.
